Extended Trip Form

Extended Trip/Temporary Withdrawal Process:

On or before your student’s last day at Mountain View Middle School, we have created this checklist for your convenience to remind your student what needs done on their final day of school.

On your last day:
1. Return Textbooks to your teachers including: Language Arts, Math/Algebra/
Geometry, Reading, Science, Social Studies, and French/German/Spanish.
2. Library- return borrowed books.
3. Cafeteria- get remaining lunch account money from Food Services at the District Support Facility.
4. Medication- contact the school nurse and arrange to pick it up.
5. District Issued Devices - please return BOTH your charger, as well as your
laptop or Chromebook to the Counseling Suite during 9th
period on your final day of school. If you have a District Issued Hotspot we
will need this returned at the same time.

Withdrawal Process

Families who wish to withdraw their student from Mountain View Middle School should complete the following:

1a)  If moving within the United States, the new school will need to contact Mountain View Middle School with their release forms to officially withdraw the student from and have the appropriate records sent. Call 717-697-1960. 

1b)  If moving outside the country, please mail a letter to the Mountain View Office at the following address:

Please note that records will not be sent until your student is fully registered at the new school and we have received a request for records.  Any student under 18 years of age is under PA compulsory attendance laws and must be enrolled in school.

On or before your student's last day at Mountain View, your student must:

1. Return borrowed books to each teacher.

2. Return borrowed library books.

3. Get remaining lunch account money from the cafeteria by contacting Yolanda Hack at yhack@cvschools.org at CV Food Services.

4. If necessary, if your student takes medication at school, you should contact the school nurse to pick it up.

5. On your student's final in-person school day please have them turn in their district issued device and charger to Mrs. Meyer in the counseling suite.

Work Permit

Work Permit (ages 14 to 17)

To obtain a work permit, a student must provide a valid proof of age and an application (completed by a parent/guardian).

Acceptable proof of age includes: Birth certificate, Military ID, Passport/Passport card, Baptismal Certificate, Driver’s License/Learner's Permit.

Applications may be completed and printed from the CV Website (search under High School > Forms & Publications > Work Permits).

Email completed application & proof of age to:workpermits@cvschools.org. Once paperwork has been received the work permit will be mailed to the address provided.
